Coroner opens inquiry into death of 78-year-old pedestrian; driver claims she did not hit her

SINGAPORE: A coroner’s inquiry opened on Wednesday (Apr 1) into the death of a 78-year-old pedestrian that occurred in unusual circumstances. Before she died, Madam Koh Kuan claimed she was struck by a car, but the driver – who took Mdm Koh to the hospital – said her vehicle did not have any impact with the woman. The sons of the late Mdm Koh, a Singaporean, attended the inquiry with a lawyer and asked the investigation officer multiple questions including about the inspection of the driver’s vehicle, which had some damage to the front that the driver claimed to be “old”. The court heard that the driver, Ms Monica Dee Choo-Ng Jolivet, a 54-year-old housewife, had lodged a police report at about 11am on Dec 31, 2024.  She said that she had been travelling along Selegie Road and making a right turn onto Sungei Road when she saw that Mdm Koh was on the road after a fall. The incident is believed to have taken place on Dec 31, 2024. Ms Choo-Ng then took her …

Unclear whether Ricky Hatton meant to take own life, coroner rules | UK News

Boxer Ricky Hatton had brain disease caused by repeated head trauma when he died, an inquest has ruled. Alison Mutch, senior coroner for South Manchester, recorded a verdict of hanging, saying he was the person who caused his own death. But she did not come to the conclusion it was suicide, saying it was unclear whether the former world champion meant to take his own life. The inquest, in Stockport, was told that Hatton was more than twice the alcohol drink drive limit at the time of his death, aged 46, in September last year. Ms Mutch said police found no notes or anything that gave any suggestion of an intent to take his life. He had been under the influence of alcohol and there were also brain changes identified by the pathologist at his post mortem as chronic traumatic encephalopathy (CTE) – a brain disease caused by repeated head trauma in contact sports like boxing. British woman, 23, shot dead by dad in US after Trump row was unlawfully killed, coroner rules | UK News

British woman, 23, shot dead by dad in US after Trump row was unlawfully killed, coroner rules | UK News

A 23-year-old British woman who was shot dead by her father while visiting him at his home in the US was unlawfully killed, an inquest has concluded. Lucy Harrison, a fashion buyer from Warrington, Cheshire, was staying with her father, Kris Harrison, and his family at their home in Prosper, Texas, in January last year. Mr Harrison, described to the court as a functioning alcoholic, claimed the gun had gone off accidentally in the incident, shortly before 3pm. Concluding the inquest into Ms Harrison’s death on Wednesday, senior coroner Jacqueline Devonish said she found the father’s actions “to be reckless”. She said that “to shoot [his daughter] through the chest whilst she was standing would have required him to have been pointing the gun at his daughter, without checking for bullets, and pulling the trigger”. The coroner found Ms Harrison died due to unlawful killing on the grounds of gross negligence manslaughter, adding that the 23-year-old was “young, vibrant and beautiful with her whole life ahead of her and this is a most tragic death”. …

Heading football ‘likely’ to have contributed to death of former Man Utd star, coroner finds | UK News

Heading football ‘likely’ to have contributed to death of former Man Utd star, coroner finds | UK News

Heading a football is “likely” to have contributed to the brain injury which was a factor in the death of former Manchester United and Scotland defender Gordon McQueen, a coroner in North Yorkshire has found. The central defender, who died at his home in North Yorkshire in 2023 aged 70, won 30 international caps between 1974 and 1981 and also played for Leeds United during a 16-year career. Appearing at a hearing in Northallerton, North Yorkshire, earlier this month, TV presenter Hayley McQueen said her father had previously told his family that heading a football for so long “probably hasn’t helped”. She said he was diagnosed with vascular dementia in 2021. The cause of death was pneumonia as he had become frail and bed-bound for months, the inquest previously heard. That frailty was due to a combination of vascular dementia and chronic traumatic encephalopathy (CTE), coroner Jon Heath said. ‘Missed opportunity’ to take footballer who died of flesh-eating infection to hospital, coroner rules | UK News

‘Missed opportunity’ to take footballer who died of flesh-eating infection to hospital, coroner rules | UK News

There was a “missed opportunity” to take an amateur footballer who died of sepsis and necrotising fasciitis – a rare flesh eating infection – to hospital, a coroner has ruled. Sophie Lomas said it was not clear whether earlier treatment would have changed the outcome of his case, however. Luke Abrahams, 20, from Northampton, returned home from a Sunday match complaining of a sore throat in January 2023. His inquest at the town’s Guildhall heard how he was prescribed antibiotics for tonsillitis, but later that week he developed severe leg pain and an ambulance was called. Paramedics diagnosed him with sciatica and did not take him to hospital, in spite of his pain level registering a “score” of nine out of 10, having a raised heart rate, dark-coloured urine, and high blood sugar levels. Giving evidence, Susan Jevons, head of patient safety at East Midlands Ambulance Service, said the crew had failed to properly consider infection, despite multiple “red flags”. Luke’s condition continued to worsen, and two days later he was taken to hospital by …

Police who named wrong fatal car crash victim ‘used family description and ID card’, coroner hears | UK News

Police who named wrong fatal car crash victim ‘used family description and ID card’, coroner hears | UK News

A police officer who wrongly identified the victim of a car crash relied on the family’s description of him and a photo ID card when he tried to establish who he was, a coroner has been told. South Yorkshire Police (SYP) admitted this week that Joshua Johnson, 18, died following a collision on Todwick Road, Rotherham, in December, along with a 17-year-old girl. The force had originally said Trevor Wynn, 17, was the male killed in the incident and Mr Johnson was badly injured in hospital. But then SYP referred itself to the Independent Office for Police Conduct (IOPC) this week after new “information came to light”, which had prompted further identification processes being undertaken. The IOPC is investigating how the misidentification occurred, promising to “carefully examine” whether procedures were “followed appropriately”. Image: Joshua Johnson.
