A young girl lies in a hospital bed in Taiz, southwest Yemen, recovering from surgery to treat her atrial septal defect (ASD), better known as “hole in the heart”. “May I take a picture of you?” a visitor asks. She smiles, slowly adjusts her position, and gets ready for the photo. Ten-year-old Noor Majid has had ASD since birth. Her condition leads to constant breathing problems and chronic exhaustion. It is hoped the surgery will help her live a life similar to that of other girls her age. Noor was one of 110 children from different parts of Yemen being treated for free at the Cardiac and Vascular Diseases and Kidney Transplant Center in Taiz between May 16 and 21. More than half of transplant recipients in a large analysis developed chronic graft-versus-host disease, and 15% died from causes other than cancer relapse. Those numbers capture the uneasy truth of allogeneic hematopoietic cell transplantation. The cure can come with a long, uncertain tail. Chronic graft-versus-host disease, or cGVHD, happens when donor immune cells attack the patient’s healthy tissues. It can injure the skin, eyes, mouth, joints, and lungs. Moreover, it remains a leading cause of debilitating illness and nonrelapse mortality after transplant. A research team led by Sophie Paczesny, M.D., Ph.D., at MUSC Hollings Cancer Center, worked with Michael Martens, Ph.D., and Brent Logan, Ph.D., at the Center for International Blood and Marrow Transplant Research at the Medical College of Wisconsin. The team set out to catch that danger earlier. Their new work in the Journal of Clinical Investigation describes BIOPREVENT, a machine learning tool designed to estimate a patient’s future risk of developing cGVHD and of dying without relapse. A two-year-old boy in Italy who received a severely damaged donor heart during a transplant has died, prompting a criminal investigation and widespread outrage. Domenico, who had been on life support for nearly two months, died shortly before 09:30 local time on Saturday, February 21 at Monaldi Hospital in Naples, according to the family’s lawyer, Francesco Petruzzi. The hospital said the toddler suffered a “sudden and irreversible worsening of his clinical condition.” The heart transplanted into Domenico in late December was reportedly transported in direct contact with dry ice, causing extensive tissue damage. Prosecutors have opened an investigation into the case, and six medical professionals are under formal investigation. Petruzzi said the organ arrived “burned by frostbite” after being transported more than 800 kilometres from Bolzano to Naples in an unsuitable container, without a thermometer to monitor the temperature.
